Required if:

  • Form 122A-1Supp: Statement of Exemption from Presumption of Abuse Under Section 707(b)(2)

  • Form 122A-2: Ch. 7 Means Test Calculation

• Chapter 11:

• • Form 122B: Ch. 11 Statement of Your Current Monthly Income

• Chapter 13:  

• • Form 122C-1: Ch. 13 Statement of Your Current Monthly Income and Calculation of Commitment Period

  • Form 122C-2 Ch. 13 Calculation of Your Disposable Income

• Means Test must be filed with the Voluntary Petition or within 14 days of filing.

Step-by-Step Procedures:

1. Select [Bankruptcy > Misc/Other].

2. Enter case number (e.g., xx-xxxxx).

3. Select from the list the applicable event(s):
   
   [Chapter 7 Statements - Monthly Income 122A-1/Exemption Presumption of Abuse (122A-1Supp)].
   
   [Chapter 7 Means Test Calculation Form 122A-2], if applicable.
   
   [Chapter 11 Statement of Monthly Income Form 122B].
   
   [Chapter 13 Statement of Monthly Income Form 122C-1].
   
   [Chapter 13 Calculation of Disposable Income Form 122C-2].

4. Select the party filer.

5. Browse to attach the Means Test (PDF file).

6. Answer question re: Presumption of Abuse (Chapter 7 only).

7. Modify docket text if appropriate.

8. Verify the final docket text; if correct, click [Next] to submit your document(s).
   
   The Notice of Electronic Filing displays, giving you the document number. Copies of this notice are immediately emailed to all participants who receive electronic notification in the case.
