Chapter 13 Plan

Filing Requirements:

• Chapter 13 Plan.   Chapter 13 Plan (fillable PDF)
                             Filing the New Chapter 13 Plan Form
                             Liquidation Analysis (PDF)
                             Liquidation Analysis (Excel)
  
  Chapter 12 or 13 plan amendments must be made by filing the entire plan as amended. Plan amendments must be titled "First Amended Plan," "Second Amended Plan," etc., as appropriate.

• Review plan and check appropriate boxes in event for:
  
  - Request to Avoid a Lien (Part 1.2)
  - Request to surrender collateral (Part 15)
  - Request to assume a lease or executory contract (Part 5).   

• Proof of Service on all creditors and parties of interest.

1. Select [Bankruptcy > Plan].

2. Enter the case number (e.g., xx-xxxxx).

3. Select [Chapter 13 Plan] from the event list.

4. Select the attorney.

5. Select the party filer.

6. Does this plan (PDF) include the Proof of Service? Insert (y or n).
   
   The Plan and Proof of Service may be a single PDF; or the Plan and Proof of Service may be filed as separate documents. The Plan must be the first document in your PDF.

7. Browse to select the Plan (PDF file).

8. Select the requests for relief that are included in the Chapter 13 Plan.  Click [Next] if none of these requests are included in Plan)

11. Modify docket text if appropriate.

12. Verify the final docket text; if correct, click [Next] to submit your document(s).
    
    The Notice of Electronic Filing displays, giving you the document number. Copies of this notice are immediately emailed to all participants who receive electronic notification in the case.
