Filing Requirements:

• Notice of Voluntary Conversion by Debtor to Chapter 7 from Chapter 13/Chapter 12.

• Filing Fee  [Note:  Refer to Bankruptcy Court Miscellaneous Fee Schedule for exceptions to filing fee. 28 U.S.C. Section 1930].

• No Proposed Order necessary (form order issued unless requested by Court)

To File Electronically:

1. Log into CM/ECF.

2. Select [Bankruptcy >  Notices].

3. Enter the case number (e.g, xx-xxxxx).

4. Select [Convert Case xx to 7 Voluntarily by Debtor]
   (for Ch 13 and Ch 12 only, when NO motion filed) from the event list.

5. Select the party filer.

6. Browse to select the document (.PDF file).
   
   Note:  The Proof of Service may be added as an attachment to this document.

7. To pay via ACH or credit card, answer "y" to "Are you paying via Internet?" Select [Next] to continue. The dollar amount will appear on the next screen.

8. Modify the text as appropriate.

9. Verify the final docket text; if correct, click [Next] to submit your document(s).
   
   Note: The Notice of Electronic Filing displays giving you the document number. Copies of this notice are immediately e-mailed to all participants who receive electronic notification in the case.
