Notice of Correction of Social Security Number or Individual Taxpayer Identification Number

When the social security number or individual taxpayer identification is incorrectly reported to the Clerk's office, a Notice of Correction of Social Security Number or Individual Taxpayer Identification Number (ITIN) is needed.

Note: Do NOT file Notice of Correction unless SSN is redacted. Mail the Notice of Correction with full social security numbers or individual taxpayer identification to the Court.

Filing Requirements:

  • Notice of Correction of Social Security Number (Do NOT electronically file this document).

    (Note: The Notice of Correction that is mailed to all creditors, U.S. Trustee and Trustee needs to contain complete social security number)

  • Proof of Service indicating notice was mailed to all creditors including U.S. Trustee and Trustee.


To File Electronically:

  1. Log into CM/ECF.

  2. Select [Bankruptcy >Misc/Other] OR [Bankruptcy>Filed by Attorney]

  3. Enter the case number (e.g, xx-xxxxx).

  4. Select [Certificate of Service] from the event list.

  5. Skip the Joint Filing screen.

  6. Select the party filer.

  7. Browse to select the document PDF file.

  8. Modify the text as appropriate.

  9. Enhance docket text to include:   "Notice of Correction not filed due to privacy act", then verify the final docket text; if correct, click [Next] to submit your document(s).

    Note: The Notice of Electronic Filing displays giving you the document number. Copies of this notice are immediately e-mailed to all participants who receive electronic notification in the case.

  10. Mail original Notice of Correction to Court.
