Payment Advices

Debtor must file copies of all payment advices or other evidence of payment received by the debtor from any employer within 60 days before filing the petition. Must be filed with the petition or within 14 days of filing. Required if debtor is an individual.

Note: When debtor is self-employed or has not received payment from an employer within 60 days before filing the petition, debtor may file Statement Concerning Payment Advices.


To File Electronically:

  1. Log into CM/ECF.

  2. Select [Bankruptcy > Misc/Other].

  3. Enter case number (e.g., xx-xxxxx).

  4. Select [Payment Advices] from the event list.

  5. Skip the Joint Filing screen.

  6. Select the party filer.

  7. Browse to attach the Employee Income Records (PDF file).

  8. Modify text as appropriate.

  9. Verify the final docket text; if correct, click [Next] to submit your document(s).

    Note:  The Notice of Electronic Filing displays giving you the document number. Copies of this notice are immediately emailed to all participants who receive electronic notification in the case.
