Note: Chapter 12 or 13 plan modifications shall be made by filing the entire plan as modified. Plan modifications shall be titled "First Modified Plan," "Second Modified Plan," etc., as appropriate. A motion to modify a plan shall include a summary of the modifications to the plan.
Reminder: Review plan and check appropriate boxes in event for:

- Request to Avoid a Lien (Part 1.2)
- Request to surrender collateral (Part 15)
- Request to assume a lease or executory contract (Part 5).

To File Electronically:

Log into CM/ECF.
Select [Bankruptcy > Plan].
Enter the case number (e.g, xx-xxxxx).
Select [Modified Plan (After Confirmation)] from the event list.
Skip the Joint Filing screen.
Select the party filer.
Browse to select the Modified Plan (.PDF file).
For Chapter 13 cases, select the requests for relief that are included in the Modified Chapter 13 Plan that were NOT included in the previous confirmed Chapter 13 Plan and are NOW included in the Modified Plan. Click [Next] if none of these requests are included in Modified Plan)
Refer to confirmed Plan.
Modify the text as appropriate, if applicable.
Verify the final docket text; if correct, click [Next] to submit your document(s).

Note: The Notice of Electronic Filing displays giving you the document number. Copies of this notice are immediately emailed to all participants who receive electronic notification in the case.
