Amended Voluntary Petition

A Voluntary Petition may need to be amended due to incorrect spelling of names, incorrect last four digits of social security number(s), county of residence, or other incorrect information contained in the voluntary petition at the time of filing.

Note:  An Amendment Cover Sheet is required to be filed with every amendment.  If the Amended Voluntary Petition is to correct the debtor(s) social security number, you must also file separately an Amended Statement of Social Security Number or Notice of Correction which includes the full SSN.

 

Step-by-Step Procedures:

  1. Log into CM/ECF.

  2. Select [Bankruptcy >  Misc/Other].

  3. Enter the case number (e.g., xx-xxxxx).

  4. Select [Amended Schedules] from the event list.  

    Note:  The Amended Voluntary Petition should only include the redacted SSN [last 4 digits only].  The Amended Statement of Social Security Number is filed separately.

  5. Select the party filer.

  6. Browse to select the Amendment Cover Sheet and Amended Voluntary Petition (PDF file).

  7. Review screen for amended summary of schedules information; click  [Next].

  8. Checkmark Voluntary Petition on list.

  9. Modify docket text if appropriate.

  10. Verify the final docket text; if correct, click [Next] to submit your document(s).

    Note: The Notice of Electronic Filing displays, giving you the document number. Copies of this notice are immediately emailed to all participants who receive electronic notification in the case.
