The proof of claim (Official
Form B410) is created after the claim information is entered
and submitted. You may upload supporting documentation.

To electronically
file claim:
From the home
page of the court's website, select [E-Filing
Proof of Claim].
Select [File a Proof of Claim]
Enter the case
number (e.g., xx-xxxxx).
Skip the "Name
of Creditor" box.
The "Filed
by" drop-down list allows you to choose your role as
filer. If you are representing the creditor, select creditor
or creditor's attorney, if applicable.
Important: Read
the redaction notice and warning. If necessary,
review any attachments you will be submitting for personal
identifiers. Check the acknowledgment box and click [Next].

Select the creditor
filing the claim by clicking the radio button by the name
or
select [Creditor Not Listed].
Note:
If the creditor is not shown with the correct address, select
"Creditor Not Listed" and fill in the correct address.

Verify the bankruptcy
case and creditor information selected or
insert information.

In order for
the Official Proof
of Claim (B410) form to be created properly, the following
information should be completed:
Amount
of Claim.
Basis
for Claim.
Last four
digits of the debtor's account (if
any).
Secured
portion of claim (if
any).
Priority
portion of claim (if
any).
Supporting
documentation (if necessary).
Creditor's
signature.
Verification
Code

If you are
attaching supporting documentation, you will be asked to browse
for the .PDF file.
Verify all
claim information entered and enter Verification Code.
Click [Submit Claim].
Receive a
confirmation screen stating claim number and a verification
email will be sent to you at the address provided.
