Filing Requirements:

To file a claim:

  1. Complete Proof of Claim form and create PDF.

  2. Log into CM/ECF.

  3. Select [Bankruptcy >  File Claims]

  4. Enter the case number (e.g., xx-xxxxx). Select [Next].

  5. Search for the creditor from the drop down list. Verify that the creditor's address is the same. If creditor is not listed or has different address, Add Creditor.

  6. Enter Proof of Claim information:

    • Amends Claim #:  (Click on "Find" if applicable).

    • Filed By:  Select appropriate Party Filer, (e.g., creditor, attorney, trustee, etc.)

    • Date Filed:  (Defaults to current date)

    • Amount Claimed:  Enter amount claimed and amount of Secured and/or Priority as listed on the claim (do not enter "$" symbol or commas in the dollar amount fields).

    • Description or Remarks: Type further description or remarks for the claim if necessary.

  7. Verify all claim information entered.  Click [Next].

  8. Browse to select the Claim (PDF file). Click [Next].

    The Notice of Electronic Filing displays, giving you the document number. Copies of this notice are immediately emailed to all participants who receive electronic notification in the case.
