Chapter 13 Filing Requirements

 

All petitions and schedules must be accompanied by the filing fee, or individual debtors may file an application to pay the filing fee in installments ($100 first installment payment is requested at the time of filing or within 14 days).  Filing fees paid directly by a debtor must be paid either by cash, cashier's check or money order made payable to "Clerk, U.S. Bankruptcy Court." PERSONAL CHECKS OR CREDIT CARDS FROM DEBTORS WILL NOT BE ACCEPTED.

  • Attorney’s Disclosure of Compensation (Form 2030) 
  • Copies of all payment advices (pay stubs) or other evidence of payment received by the debtor from an employer within 60 days before filing the petition (all but the last four digits of debtor’s social security number redacted) 
  • Chapter 13 Plan including liquidation analysis and Proof of Service (service on all creditors and interested parties) (Local form required)
 
Due no later than last payment under the debtor’s plan or, at the time a motion pursuant to § 1328(b) is filed.
