E-Filing Proof of Claim
- Proofs of Claim for all chapters may be filed electronically through the court’s website. A CM/ECF login and password are not required.
- The proof of claim (Official Form 410) is created for you after the claim information is entered and submitted. You may upload supporting documentation.
- Use "File a Claim Supplement" to file Notice of Mortgage Payment Change, Notice of Postpetition Fees, Expenses and Charges, Response to Notice of Final Cure Payment and Proof of Claim Attachment 3001 (c)(1) and (d).
- The name and complete address of the creditor must be entered. If an attorney is filing the claim on behalf of a creditor, the attorney should also enter his/her name and address. The name and title, if any, of the person authorized to file the claim is required. The name should be preceded by “/s/” as, for example,”/s/ Jane Doe”.

Penalty for filing fraudulent claim: Fine of up to $500,000 or imprisonment for up to 5 years, or both. 18 U.S.C. §§ 152 and 3571.
- Information that is entitled to privacy: A Proof of Claim form and any attached documents must show only the last 4 digits of any social security number, an individual’s tax identification number, or a financial account number, only the initials of a minor’s name, and only the year of any person’s date of birth. If a claim is based on delivering health care goods or services, limit the disclosure of the goods or services to avoid embarrassment or disclosure of confidential health care information. You may later be required to give more information if the trustee or someone else in interest objects to the claim.
